MISSION VOLUNTEER FORMS
Thank you for your interest in Canvasback's Missions and our ministry to the Micronesia Islands. Since our ministry involves International travel and work you can understand that we need a lot of information to cover the event. Please complete the following forms to meet the mission needs. Please follow the instructions provided by the DOVE representative.
Basic Form - Application
The process begins with the application. There are three forms to complete this process. The first form will create your account which will be used for the other forms. If you already have an account, you can sign in first and then fill out the first form. Some of our volunteers have experienced a few problems signing in. Most can be corrected by resetting the password. We can also help you reset the password.

Personal Medical Information
In the case of emergency we have had to use this type of information for your health and safety. Please forgive our intrusion into your personal information.

Passport Information
Traveling abroad requires a passport and we will need this information and an image of it in order to process travel paperwork. We will ask for the image later.
Medical License Information
The Islands all require medical professionals to be fully licensed. This information will be used to meet the island paperwork requirements.

Volunteer Agreement
Before you have boots on the ground we want you to know what our expectations are for your performance on-island. You will be going to a foreign country that has it's own culture and laws. These standards will protect both you and us.
Media Release
Media materials in many different forms will be collected during the mission trip. We will use the material in the course of our normal business process.
